Buckinghamshire Healthcare NHS Trust is recruiting a Band 4 Community Healthcare Assistant Practitioner to join the Southern Rapid Response and Intermediate Care team. The role involves assessing, planning, implementing, monitoring and evaluating exercise and education programmes for referred individuals, working both independently and with the multidisciplinary team.

• Assess and plan exercise and education programmes for referred individuals.
• Work independently and with the multidisciplinary team.
• Experience with patients with complex healthcare or rehabilitation needs.
• Clinical reasoning to assess and treat within service protocols.
• Training and career development opportunities including a nationally recognised care certificate.

Responsabilités

• Deliver clinical and therapeutic care within set protocols and scope of Assistant Practitioner.
• Carry a caseload of non-complex patients.
• Coordinate safe admission, transfer and discharge of patients.
• Assist patients with personal hygiene, dignity and cultural needs.
• Ensure safe and competent use of equipment and report issues.
