Walsall Council is seeking a Social Care Facilitator within the Adult Social Care Mental Health Services. The role supports adults with mental ill-health through strengths-based, person-centred social care practice, working alongside Social Workers, AMHPs and community partners.

You will conduct Care Act assessments, support planning, and reviews to promote independence and community access, with a focus on recovery and social inclusion.

Competenze

• Experience working within Adult Social Care, Mental Health Services, Care Coordination or Recovery Services.
• Knowledge of Care Act 2014 and strengths-based assessment.
• Experience undertaking assessments, reviews and person-centred care plans.
• Strong communication, organisational and report-writing skills.
• Ability to work independently and in a multi-disciplinary team.

Mansioni

• Undertake Care Act assessments to identify eligible needs and outcomes.
• Complete Carer's Assessments and Section 117 Aftercare assessments.
• Develop and review person-centred support plans and aftercare plans.
• Coordinate with CMHTs, AMHPs, NHS Trusts and partners to deliver coordinated support.
