Job summary

Bracknell Forest Council is recruiting an experienced Adult Social Worker to join the Community Team for People with Learning Disabilities (CPTLD) in Bracknell.

You will conduct assessments, develop care plans, and support service users and carers with a strengths-based, person-centred approach, ensuring safeguarding and legal compliance.

Hybrid working with minimum 2 days in the office at Time Square, Bracknell, RG12 1JD.

Qualifications

• Qualified Social Worker with Social Work England registration.
• Experience in Adult Social Care, preferably with learning disabilities.
• Strong knowledge of Care Act 2014, Mental Capacity Act 2005, safeguarding.

Responsibilities

• Conduct assessments, devise care and support plans, and review plans.
• Participate in duty arrangements and MDT meetings.
• Apply strengths-based approach and connect clients to their communities.
• Develop person-centred support plans with emphasis on independence.
• Assess and support family carers and complete RAS assessments where required.
• Ensure compliance with departmental policies and statutory requirements.
