Job summary

Leaders in Care Ltd is seeking an experienced Learning Disability Social Worker in Manchester. The role offers hybrid working with 3 days in the office and 2 days at home.

You will complete strengths-based assessments and develop person-centred care plans while coordinating with families, providers and partner agencies to maximise independence and quality of life. You must hold a recognised Social Work qualification and current Social Work England registration to apply.

Qualifications

• A recognised Social Work qualification.

• Experience working with adults with learning disabilities.
• Strong assessment, care planning and safeguarding skills.
• The ability to work effectively with families, providers and multi-disciplinary partners.
• A commitment to inclusive, strengths-based and person-centred practice.
