Job summary

Tripod Partners is seeking an experienced Adult Social Worker to join our Adult Social Care - Learning Disabilities team in Somerset. The role focuses on assessments, care planning and decision-making to support adults with learning disabilities.

You will work in a multidisciplinary setting, complete Care Act assessments, and travel across Somerset. Hybrid working with two days on site and some travel within the county are expected.

Qualifications

• Qualified Social Worker with Learning Disability experience.
• Experience in Care Act assessments and adult social care.
• Ability to travel across Somerset as required.

Responsibilities

• Complete Care Act assessments, reviews and reassessments for adults with care and support needs.
• Work within a multidisciplinary team, contributing professional social work expertise.
• Identify and evidence eligible care and support needs in accordance with legislation and guidance.
• Undertake robust, evidence-based assessments and demonstrate analytical skills.
• Exercise professional judgement and make defensible, proportionate decisions.
• Collaborate with Social Care, Health Services, and other partner agencies.
• Contribute to CHC processes and checklists where applicable.
• Maintain accurate records and use IT systems for information management.
• Travel across Somerset and attend the office at least two days per week.
