Job summary

Waythrough is seeking a Housing Support Coordinator in Darlington to deliver person-centred housing support and help people secure and maintain accommodation. You will manage a caseload, complete needs and risk assessments, and provide duty cover for the drop-in service, including outreach in homes.

The role requires an NVQ Level 3 in Health & Social Care (or equivalent), strong communication and organisation skills, and a valid UK driving licence.

Qualifications

• NVQ Level 3 in Health & Social Care or equivalent.
• Proven communication and interpersonal skills.
• Willing to work towards relevant qualification if not held.

Responsibilities

• Provide duty cover for users of the drop-in service.
• Manage a caseload and complete needs and risk assessments.
• Offer outreach support in individuals' homes.
• Work with partnership agencies to ensure continuity of care.
• Promote safeguarding and wellbeing of those supported.
• Maintain accurate records and plans.
