Single Homeless Project in London is seeking an Outreach Housing Pathway Coordinator to ensure rough sleepers access and sustain suitable accommodation. You’ll work within Camden’s integrated Rough Sleeping and Outreach Hub, tracking where each person is in their housing journey and coordinating with outreach teams, hostels and statutory services.

Qualificações

• Experience in homelessness services or social care.
• Ability to coordinate multi-agency pathways and manage complex cases.
