Job summary

THE SINGLE HOMELESS PROJECT is seeking an Outreach Housing Pathway Coordinator in Camden to help rough sleepers move into suitable accommodation and sustain housing. You will work across street outreach, hub services and partner agencies to remove barriers, progress housing options, and ensure timely, safe placements.

This role requires persistence, practical judgement and experience coordinating complex cases, safeguarding and risk management, within Camden’s No Wrong Door model to support

Qualifications

• Knowledge of safeguarding frameworks and risk management processes.
• Experience using case management systems such as CHAIN.
• Experience of multi-agency case coordination and partnership working.
• Experience of assertive engagement with individuals with complex needs.
• Significant experience working with people experiencing rough sleeping or multiple disadvantage.
• Knowledge of homelessness, housing and anti-social behaviour legislation.

Responsibilities

• Coordinate access to Camden’s Adult and Young People’s Pathways, ensuring eligible individuals move efficiently from rough sleeping into appropriate accommodation.
• Support referrals between pathway projects and accommodation providers, ensuring placements are timely, appropriate and sustainable.
• Monitor cases where tenancy sustainment or behaviour may place accommodation at risk, working proactively with hostels and partners to prevent eviction and return to the streets.
• Maintain oversight of pathway “holding spaces” and transitional placements to ensure effective use of accommodation resources.
• Work closely with outreach and hub teams to support rapid progression from street engagement into accommodation pathways.
• Provide assertive engagement with individuals who may be reluctant to access support or accommodation.
• Ensure pathways are accessible to individuals with complex needs, including those experiencing substance use, mental ill health or multiple disadvantage.
• Support coordinated responses where rough sleeping and street activity intersect with accommodation provision.
