Job summary

Leicestershire County Council is seeking Community Support Workers to support hospital discharge and facilitate safe, person-centred plans for patients returning home or moving to a care setting in Leicestershire. Based at Leicester Royal Infirmary and across three acute hospitals, you will work with therapists, nurses, and families to improve outcomes.

This permanent role includes a structured progression from Grade 8 to Grade 9, with an inductive programme, Care Certificate, and ongoing

Qualifications

• Level 2 qualification (or equivalent) in Maths and English and a relevant health/social care qualification or equivalent experience
• Understanding of adult social care needs and risk assessments
• Ability to undertake direct work with those who access or need the service
• Strong communication, teamwork, and partnership working skills
• Demonstrated ability to manage caseloads and deliver person-centred care
• Good written and verbal communication and negotiation skills
• The ability and means to travel across the county of Leicestershire to undertake visits

Responsibilities

• Support people in hospital as they prepare to leave inpatient care or move to a care setting for rehabilitation and reablement
• Work within a multidisciplinary team to shape safe, person-centred discharge plans
• Progress through a career-graded route from Grade 8 to Grade 9 with mentoring and development
• Undertake assessment, support planning and case management as responsibilities expand
• Ensure DBS checks are up to date and maintain safeguarding standards
