Leicestershire County Council invites applications for a Community Support Worker in the Hospital Discharge Team. Based at Leicester Royal Infirmary, you will help plan safe discharges, working with therapists, nurses, doctors, and families to support patients returning home or to care settings.

The role includes a structured progression from Grade 8 to Grade 9, with a Learning and Induction Programme and Care Certificate, enabling you to take on more complex assessment and care planning tasks.

Qualificações

• Level 2 Maths and English qualifications or equivalent.
• Understanding of adult social care needs and risk assessments.
• Ability to travel across Leicestershire for visits.

Responsabilidades

• Support discharge planning and multi-disciplinary teamwork.
• Assist in creating person-centred discharge plans with families.
• Maintain and review caseloads; provide compassionate care.
