Job summary

Leaders in Care is recruiting a Community Care Social Worker to join a Pathway 3 discharge service. You’ll coordinate discharge plans, work with patients, families, health colleagues and community providers to ensure safe, timely hospital discharge and appropriate ongoing support.

The role offers a fast-paced, flexible environment with hybrid working (1–2 days in the office) and a busy, varied caseload within a supportive team.

Qualifications

• A recognised Social Work qualification.
• Active Social Work England registration.
• Experience in hospital discharge, community care or adult social care is desirable.
• Ability to manage a fast-paced caseload and undertake community visits.

Responsibilities

• Complete strengths-based assessments and care planning.
• Coordinate discharge arrangements and onward community support.
• Manage safeguarding and risk appropriately.
• Help prevent delayed discharges while ensuring safe outcomes.
• Work collaboratively across health and social care services.

The role
