Job summary

Leeds City Council is seeking a Social Worker (Mental Health Hospital Discharge) to support safe, person-centred discharge from inpatient mental health services. The role is based at The Mount Hospital with hybrid working and regular ward reviews.

You will manage a varied caseload, undertake Care Act assessments, coordinate discharge plans and work with NHS colleagues, families and carers to promote independence and recovery.

Qualifications

• A recognised social work qualification and Social Work England registration.
• Experience with adults with mental health needs and relevant legislation (Care Act, Mental Health Act).
• Strong assessment, risk management and care planning skills.

Responsibilities

• Carry a caseload and undertake assessments using a person-centred approach.
• Devise and implement service and support plans based on assessed needs and risks.
• Monitor and review plans; maintain case notes and written reports; provide evidence in court if required.
