Job summary

Northern Care Alliance NHS Foundation Trust is seeking a Patient Flow Co-ordinator to support clinical site teams in rapid discharges and correct patient placements. You will work with bed, ward and operations managers to ensure smooth patient flow and timely escalation of capacity pressures.

You will review potential discharges, monitor bed and infection control status, and assist with internal moves and hospital repatriations as required.

Qualifications

• Experience using data to measure performance and plan resources.
• Ability to work under time pressure and respond to urgent requests.
• Experience managing patient flow or pathways in an NHS environment.

Responsibilities

• Support bed managers, ward managers and operational managers to allocate patients appropriately to clinical areas.
• Identify potential discharges and ensure timely escalation when delays occur.
• Monitor bed position, side room availability and infection control issues affecting flow.
• Prepare site meetings with bed base, demand, and escalation triggers for divisions.
