Chelsea and Westminster Hospital NHS Foundation Trust is seeking a Band 4 Non Clinical Discharge Coordinator to enhance patient flow from admission to discharge. You will liaise with wards, social services, and community care to progress the patient journey and meet Estimated Dates for Discharge.

You will attend MDT and board rounds, complete Early Discharge Notifications within 24 hours of admission, coordinate Needs Based Assessments, and escalate delays to ensure safe, timely discharges

Qualifications

• Experience in discharge planning and patient flow management.
• Strong liaison with wards, social services and community care.
• Ability to escalate delays and act as a point of contact for MDTs.

Responsabilités

• Facilitate timely discharge across hospital pathways.
• Attend Daily Reason to Reside MDT meetings and board rounds.
• Ensure Early Discharge Notifications are completed within 24 hours of admission.
• Coordinate Needs Based Assessments and referrals ahead of discharge (ADD).
• Confirm and document packages of care on Cerner/Optica and handover to clinical teams.
• Support flexible shift patterns including weekends and holidays.
