Brighton/Suncrest Hospice seeks a Registered Nurse to provide comprehensive hospice care across home, SNF, and assisted living settings. You will join an interdisciplinary team, offering psych-social assessments and coordinating with case management, admissions, or resource roles.

The role requires active RN licensure, strong communication, and the ability to travel within the service area. A supportive, team-oriented environment and opportunities for professional growth are highlighted.

Qualificações

• Current RN licensure (in good standing)
• Collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership
• Work closely with patients on a 1-1 level with strong support from a patient care team
• Ability to work independently with phone, email, and written communication
• Time management to manage visits, documentation, meetings, and urgent needs
• Ability to learn an electronic medical records system
• Willingness to drive with reliable transportation and travel across the service area

Responsabilidades

• Provide care for a group of patients, visiting patients where they reside (homes, SNFs, assisted living)
• Work as part of the interdisciplinary team with expertise on psych-social assessments
• Take on Case Management, Admissions, or Resource roles as needed
