Job summary

UnitedHealth Group is seeking a Nurse Case Manager to guide patients and families through the care continuum, assess needs, coordinate care, support discharge planning, and manage outpatient care under supervision. You’ll collaborate with a Care Advocate and Social Worker to ensure appropriate services and education.

Role includes on-call responsibilities, supervising LPNs and HHAs, and ongoing clinical development, with a comprehensive benefits package and opportunities for professional growth

Qualifications

• Current RN license (state-specific)
• Valid CPR certification
• 1+ years clinical RN experience
• Valid driver’s license with reliable transportation and liability insurance

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care
• Makes referrals to other disciplines as indicated by patient needs and documents rationale if not referred
• Promotes patient health and independence through teaching and rehabilitative measures
• Supervises LPNs and HHAs and communicates progress to the clinical manager and care team
• Completes documentation timely and accurately and participates in ongoing education
