Job summary

Emmaus Hull & East Riding is seeking a Homelessness Support and Progression Worker to provide structured, person-centred support in our on-site housing services. The role delivers pathways to independent living, manages tailored support plans and evidence, and collaborates with housing partners to secure move-on solutions.

The position offers a salary of £26,000–£27,000 per annum, full-time hours, and an on-site work location.

Qualifications

Experience working with vulnerable adults (e.g. homelessness, mental health, substance use).
Experience delivering person-centred support planning and progression-focused outcomes.
Ability to coordinate and support individuals towards independence and move-on.
Experience of risk assessment and management.
Strong safeguarding knowledge and professional boundaries.
Confident IT skills with case management systems.

Responsibilities

Coordinate referral, assessment and onboarding, including initial needs and risk assessments.
Deliver structured, person-centred support through 1:1 sessions and day-to-day engagement.
Co-produce, coordinate and regularly review individual support and progression plans.
Support housing, health, finances, and life skills to meet pathway goals.
Maintain accurate, timely records of all support delivered and evidence in the system.
Collaborate with housing providers to move companions toward independent living.
