Job summary

Norfolk Community Health and Care NHS Trust in Diss invites applications for a Band 5 Community Nurse to provide home-based care, assessment and interventions within the South Norfolk Health Improvement Partnership Team (SNHIP).

You will deliver palliative and wound care, support patients and carers, and work with a multidisciplinary team across community settings, clinics and homes in the Diss area, including weekends where required.

Qualifications

• Registered nurse with skills in palliative/end-of-life care and wound care.
• Ability to assess and plan, implement and evaluate patient care.
• Excellent communication skills and ability to work in multidisciplinary teams.
• Must hold a full driving licence and have access to a vehicle.

Responsibilities

• Assess, prescribe and deliver interventions for patients (long-term conditions, palliative care, rehab).
• Develop individualized care plans after holistic assessment.
• Coordinate with GPs, social services, inpatient teams and other agencies.
• Complete patient records and use SystmOne daily.
• Contribute to team meetings and support junior staff.
