Job summary

Wye Valley NHS Trust seeks a Band 5 community nurse to provide in-home care across the north-west locality. The role focuses on holistic assessment, wound care, and managing long-term conditions to help patients stay independent and out of hospital.

Travel between sites is part of the role, with shifts across mornings and evenings, including regular weekends. NMC registration is required to apply, and IT skills are advantageous.

Qualifications

• NMC registration is required.
• Nursing experience is required.
• Good communication skills are essential.

Responsibilities

• Provide treatment and support to patients in their own homes, encouraging independence to prevent hospital admissions
• Carry out holistic nursing assessments
• Wound care, including NPWT
• Wound assessments and management
• Palliative care
• Managing end-of-life symptoms and supporting relatives and carers
• Managing long-term conditions in the community
• CVAD and drain management
• Chemotherapy disconnection
• Post-operative care following discharge
• Taking venous blood samples
