Job summary

Norfolk Community Health and Care NHS Trust in Norwich is seeking a registered nurse to join the North Norwich Community Nursing Team. You will provide nursing care and interventions across Norwich Place, based at Norwich Community Hospital, with shifts 8am–6pm including weekends and bank holidays.

Applications are welcomed from experienced and newly qualified registered nurses. A supernumerary period is available and you will be supported with training and supervision.

Qualifications

• Registered Nurse with current NMC registration.
• Experience in community/home-based nursing desirable.
• Willingness to work flexible shifts including weekends/bank holidays.

Responsibilities

• Assess, plan, and deliver care packages for patients in the community.
• Collaborate with the integrated care team to facilitate discharge and prevent admissions.
• Coordinate with other healthcare professionals and agencies to provide seamless care.
• Process referrals through clinical assessment and prioritise responses.
• Communicate with patients, families, and multi-agency teams on a daily basis.
• Provide clinical assessment for incoming nursing referrals.
