Job summary

East of England Community Health and Care NHS Trust in Diss is seeking a Band 5 Community Nurse to join the South Norfolk Health Improvement Partnership Team. The role involves assessing and delivering high-quality interventions to patients primarily cared for at home or in care settings.

You will provide palliative and wound care with a focus on quality of life, require a full driving licence, and work flexibly across a seven-day week to support home-based care.

Qualifications

• Registered nurse with current NMC registration and nursing qualification.
• Experience or willingness to work in community/home-based settings.
• Ability to perform holistic assessments and plan individualized care.

Responsibilities

• Assess, plan, implement and evaluate patient care in home, care homes, and clinics.
• Deliver evidence-based interventions for long-term conditions, palliative care and rehabilitation.
• Collaborate with GPs, social services, and multidisciplinary teams to coordinate care.
