Job summary

Hampshire County Council in Winchester is seeking a qualified Occupational Therapist to join our Reablement Service on a permanent basis. You will deliver thorough home assessments, create tailored intervention plans, and review progress with patients in their own homes.

This role involves collaboration with a multidisciplinary team, providing guidance to colleagues, and working across seven days with weekend rotation. UK HCPC registration required.

Qualifications

• Qualified Occupational Therapist with HCPC registration.
• Understanding of the statutory duties of local government in social care.
• Some experience supervising staff.
• Experience in assessment, planning and reviewing case work.
• Ability to work independently and collaboratively, escalating cases as required.
• Commitment to demonstrating our values and behaviours.

Responsibilities

• Lead on assessing, planning, and reviewing complex cases for high-quality outcomes.
• Deliver person-centred functional assessments and interventions to promote independence.
• Collaborate with a multidisciplinary team of professionals.
• Provide expert guidance and support to colleagues, underpinned by social care legislation and best practice.
• Contribute to a seven-day-a-week service with weekend rotation (approx. 1 in 8 weekends) and some bank holidays.
