Job summary

Hampshire County Council in Totton is seeking a qualified Occupational Therapist with HCPC registration to join our Reablement Service. You will carry out home visits, deliver holistic assessments, and develop tailored intervention plans to promote independence across the West Hampshire area.

This permanent hybrid role is 37 hours per week, with weekend rotations and some bank holidays. You will work within a multidisciplinary team to ensure high-quality outcomes in social care practice.

Qualifications

• HCPC registered Occupational Therapist.
• Knowledge of statutory duties of local government in social care and the social care model.
• Experience supervising staff.
• Experience in assessment, planning and reviewing of casework.
• Ability to work independently and collaboratively, escalating cases as required.
• Commitment to demonstrating our values and behaviours.

Responsibilities

• Lead assessment, planning and review of complex cases.
• Deliver person-centred functional assessments and interventions.
• Collaborate with a multidisciplinary team of professionals.
• Provide autonomous clinical guidance to colleagues.
• Work within a seven-day service with weekend rotations (approx. 1 in 8 weekends) and some bank holidays.
