Job summary

Community Care Live in the United Kingdom is seeking a qualified Occupational Therapist with HCPC registration to join our Reablement Service. You will carry out home visits, deliver holistic assessments, and develop tailored intervention plans to help people regain independence.

Work in a friendly, multi-disciplinary team, with weekend rotation and some bank holidays. You’ll support safe discharges and promote person-centred care in line with social care practice.

Qualifications

  • Occupational Therapy qualification and HCPC registration.
  • Understanding of the statutory duties of local government in social care.
  • Experience supervising staff.
  • Experience in assessment, planning and reviewing of case work.
  • Ability to work independently and collaboratively, escalating cases as required.

You’ll work collaboratively within our multi-disciplinary Reablement Service - a strengths-based, short-term service designed to help people maintain or regain independence in their daily lives.

  • Take the lead on assessing, planning, and reviewing complex cases, ensuring high-quality outcomes.
