Job summary

Hampshire County Council is seeking a HCPC registered Occupational Therapist to join the Reablement Service in a hybrid role covering the West Hampshire area, including Totton. You will visit clients at home, deliver holistic assessments, develop tailored intervention plans and monitor progress to promote independence.

You will collaborate with a multidisciplinary team and work seven days a week on a rotational basis, with some weekend and bank holiday work, in a supportive environment focused

Qualifications

• Occupational Therapy qualification and HCPC registration required.
• Experience in assessment, planning and review of case work.
• Ability to work independently and with a multidisciplinary team.

Responsibilities

• Lead on assessing, planning, and reviewing complex cases with high-quality outcomes.
• Deliver person-centred functional assessments and interventions to promote independence.
• Collaborate with a diverse team of professionals and support staff.
• Provide expert guidance to colleagues in line with social care legislation and best practice.
• Contribute to a seven-day service with rotational weekend work and bank holidays.
