Job summary

Lincolnshire Community Health Services NHS Trust is recruiting a Promoting Independence Therapy Assistant for Gainsborough. You will deliver rehabilitation and reablement to patients at home after hospital discharge, supporting independence and coordinating with a multi-disciplinary team.

The role covers the Discharge to Assess pathway across Lincolnshire, with shifts between 0800 and 2000, seven days a week, and part-time options available.

Responsibilities

• Provide rehabilitation and reablement to patients immediately following hospital discharge in their home.
• Promote independence and contribute to assessments to determine future needs.
• Work closely with the multi-disciplinary team to promote independence and prevent dependence.
• Undertake environmental assessment within the person’s home.
• Communicate effectively with patients, carers, health professionals and outside agencies where appropriate.
• Participate in the delivery of complex care as prescribed by the qualified practitioner.
• Plan and progress simple patient interventions with autonomous assessing and planning on a daily basis with indirect supervision of a qualified practitioner.
• Be responsible for the safe use of all equipment within their scope of practice.
• Identify needs and advice on equipment and resources, requisitioning where there is delegated authority and ensuring effective ongoing use of equipment.
