Lincolnshire Community Health Service (LCHS) aims to recruit an Assistant Practitioner to join Adult Community Therapy across the South East locality (Spalding, Boston, Bourne and surrounding areas).

The role involves conducting holistic assessments in people’s homes or care homes, coordinating care with therapists and assistants to support rehabilitation and independence. Some travel across the county is required.

Qualifikationen

• Foundation degree or equivalent required.
• Therapy experience preferred but not essential.
• Excellent communication and teamwork skills.

Aufgaben

• Independently assess and treat patients in home or care home settings.
• Co-ordinate care with registered therapists and therapy assistants.
• Develop rehabilitation plans addressing physical, cognitive and social needs.
