Job summary

Lincolnshire Community Health Services NHS Trust is looking for an Assistant Practitioner to join Adult Community Therapy across Spalding, Boston, Bourne and surrounding areas. You will carry out home or care home assessments, coordinating care with registered therapists and therapy assistants to maximise patient independence.

You will hold a foundation degree or equivalent and travel across the county, with therapy experience preferred but not essential.

Qualifications

• Foundation degree or equivalent required.
• Experience in therapy or rehab is preferred but not essential.
• Good communication skills required.

Responsibilities

• Carry out home or care home assessments and coordinate therapy care.
• Work within an MDT to deliver high quality, patient-centred care.
• Manage a caseload and identify therapy needs while travelling around the county.
