Job summary

British Red Cross is seeking a dedicated Support Worker to join the Home from Hospital team in Surrey. This role focuses on emotional and practical support for individuals recently discharged from hospital, helping them regain independence at home.

You will manage a caseload, conduct welfare calls, and support recovery with home visits. A full UK driving licence is required, and flexible remote/hybrid working options are available.

Qualifications

• Ability to provide emotional and practical support.
• Capable of managing a caseload and coordinating welfare calls.
• Willingness to work both independently and as part of a team.
• Full UK driving licence and access to a vehicle.
• Basic IT skills and a commitment to following procedures.

Responsibilities

• Manage a caseload to provide emotional and practical support for service users discharged from hospital including welfare calls, shopping, and home visits.
• Support adults after a hospital stay to help them recover safely at home.
• Respond to referrals from clinicians and community health professionals.
• Assess needs and follow a personalised support plan for each service user.
