Job summary

Wiltshire Council's Reablement team is hiring Reablement Support Workers to assist individuals at home after hospital discharge. You will help customers achieve goals and work with occupational therapists to develop personalised reablement plans.

A genuine passion for helping others is essential; you will work on a rota 7am-10pm, 365 days a year, with travel to client visits requiring a UK driving licence and access to a vehicle. NVQ apprenticeships are available to support your upskilling.

Qualifications

• Available on a rota basis between 7am-10pm, 365 days a year.
• Valid UK driving licence and access to a vehicle, unless transport arranged.
• NVQ apprenticeships and upskilling offered.

Responsibilities

• Support individuals in their homes post-hospital discharge and help achieve their goals.
• Work with occupational therapists to co-develop personalised reablement plans.
• Travel to client visits as required.
