• Full UK driving licence held for at least 2 years and willingness to drive service vehicles
• Patient, thoughtful and able to put others at ease

Responsibilities

• Providing practical and emotional support, including welfare calls, shopping, and home visits.
• Supporting adults after a hospital stay to recover safely at home.
• Responding to referrals from clinicians and community health professionals.
• Assessing needs and following a personalised support plan for each service user.
