Responsibilities

• Oversee staff rotas and deployment to meet service demand.
• Manage a defined caseload including assessments, support planning and discharge coordination.
• Lead recruitment, induction and training, with ongoing mentoring and supervision.
• Monitor activity, outcomes and referral trends.
• Ensure safety, safeguarding, and incident learning with proper documentation.
• Collaborate with KEaH, community nursing, social care, GPs and discharge teams.
• Identify service improvements and support new care models.
