Surrey Downs Health and Care Partnership is seeking an experienced Home First Clinician to provide comprehensive care from history taking to evaluation in the Home First Service.

The post-holder will lead clinical decision-making, coordinate interdisciplinary team processes, and support prevention of admission while promoting independence and wellbeing for older adults with complex health and social needs.

Qualifications

• Assess and treat adults within the Home First Service.
• Demonstrate professional leadership and safe decision-making.
• Collaborate across the Surrey Downs Health and Care team to meet patient needs.

Responsabilités

• Coordinate assessment and treatment for adults referred from the community.
• Develop care plans and implement care provision for older adults with complex needs.
• Act as a clinical lead within the Home First Service and MDT to improve outcomes.
