Job summary

Kent Community Health NHS Foundation Trust is seeking a Healthcare Assistant to join the Urgent Care Services team and provide short-term health and care support in people’s homes across the community.

You will work alongside nurses and therapists, delivering person-centred care, supporting delegated care plans, and helping patients regain independence, with opportunities to contribute to therapy programmes and care planning. This role requires a UK driving licence and NHS experience.

Qualifications

• QCF Level 3 (NVQ Level 3 apprenticeship, BTEC or equivalent).
• Functional Skills Level 2 in English and Maths.
• Recent healthcare experience as a healthcare assistant.
• Full UK/EU driving licence.
• Care Certificate (optional; support to achieve within 3 months).

Responsibilities

• Deliver high-quality, person-centred care under the guidance of registered nurses and therapists.
• Support delegated care plans and monitor patient progress.
• Record vital signs, glucose monitoring, routine urinalysis, wound care and falls prevention support.
• Help individuals regain independence with daily activities.
• Contribute to therapy programmes and group sessions; provide feedback to clinicians.
• Follow established procedures to ensure safe, effective care and preserve dignity.
