Epsom and St Helier University Hospitals NHS Trust seeks an experienced clinician for the Home First Service. The post-holder will assess, diagnose, treat and evaluate care for older adults, leading a multidisciplinary team to prevent admission and support discharge.

You will work within a dynamic, policy-driven environment and provide expert clinical decision-making with emphasis on independence and wellbeing.

Qualifications

• Current UK professional registration (HCPC/NMC).
• Advanced physical assessment skills required.
• Desirable: Independent prescriber or working towards.
• Desirable: Extended clinical skills or working towards.

Responsabilités

• Assess and plan treatment for adults referred from community and in ED/AMU.
• Carry out holistic assessments to create care plans and reduce hospital admissions.
• Act as a professional role model; lead and develop services within the Home First Service.
• Coordinate seamless service through interdisciplinary team processes in Home First Service.
