Job summary

Epsom General Hospital, part of Epsom and St Helier University Hospitals NHS Trust, is seeking an experienced clinician for a 12-month fixed-term Band 7 post. The role involves care from initial assessment through to treatment and evaluation of care for older adults within the Home First Service.

The post emphasizes leadership, safe clinical decision-making, and collaboration with the Home First MDT to prevent admissions and support patient independence in the community.

Qualifications

• HCPC or NMC registration is required.
• Advanced physical assessment skills are required.
• Independent prescriber or working towards.
• Extended clinical skills or working towards.
• Experience in assessing and managing complex patients in community or unscheduled care.
• Multidisciplinary team work.
• Urgent care experience with telephone triage.

Responsibilities

• Clinical assessment and treatment planning for adults from community, ED and AMU.
• Collaborate with the Surrey Downs Health and Care team to deliver safe care.
• Coordinate seamless service through enhanced interdisciplinary processes.
• Lead and develop services to meet patient needs and support policy delivery.
