Worcestershire County Council is seeking an experienced Urgent Care Team Manager to lead a multidisciplinary team across Community Hospital settings, coordinating assessment, recovery-focused interventions and discharge support as part of the Home First approach.

You will work closely with NHS colleagues, Adult Social Care and providers to ensure timely, person-centred care, enabling independence and safe return home where possible.

Responsabilités

• Lead and develop a high-performing team delivering assessment and support across Worcestershire's Community Hospitals.
• Drive delivery of Home First and Discharge to Assess principles to support recovery in least restrictive settings.
• Foster partnership working with Adult Social Care, NHS, providers and community services.
• Promote strengths-based practice focusing on abilities, networks and recovery potential.
• Support reduction of unnecessary hospital stays and long-term care dependency.
• Provide leadership, supervision and professional support to staff and ensure continuous service improvement.
