Job summary

Kent Community Health NHS Foundation Trust is seeking a Band 6 Community Nursing Team Leader to oversee home-based care, prioritise patient safety, and coordinate care across home, care homes and community settings.

The role requires NMC registration, a full UK or EU driving licence, and experience managing complex caseloads. Expect 7.5-hour shifts with alternate late shifts and weekends within integrated neighbourhood teams.

Qualifications

• Level 6 degree or equivalent, or equivalent knowledge from experience plus a diploma in nursing.
• Registered with the Nursing and Midwifery Council (NMC).
• Experience in managing complex caseloads and mentoring others.

Responsibilities

• Use clinical judgement to support evidence-based care planning for people at home, in care homes and other settings.
• Keep people safe and well at home.
• Provide a good experience of services for people using the care.
• Work in integrated neighbourhood teams delivering joined-up care with GPs, mental health professionals, social workers and voluntary services.
