The shift pattern for this position is Monday- Friday, 9am-5pm.

The Home First Response Service is a service focused on working differently and collaboratively with acute and community teams to identify frailty and support people home or within a homely setting. Hospital at Home provides urgent short-term hospital level care at home. This post will be responsible for providing rehabilitation care plans to patients sitting under Home First and Hospital at Home. As part of the team we are looking for an innovative and experienced physiotherapist to join our team within the Renfrewshire Community. This post is supported with professional supervision by established structures within Renfrewshire HSCP. The focus of the team is to support frail patients with early discharge from acute settings and prevention of admission. As this is a community post, it will be managed and supervised under the Renfrewshire HSCP management structure. We would welcome applicants with an interest in Quality Improvement and Service development. Individuals will work autonomously and within the team.

The Home First Response service is a Multi-Disciplinary Team working across acute and community area identifying and working with people who live with frailty. It is both a proactive and reactive role to play in the management of frailty. The post holder will be an integral component of the patient journey from early intervention, assessment, care planning and by working proactively and collaboratively with the MDT across acute and community areas to safely discharge the patient home. The post holder will link with the community part of the HFRS team to ensure a seamless discharge and continuity of care.

Hours of work will be flexible to cover between 8am and 8pm, 7 days per week. Initially the working pattern will be Monday to Friday between the hours of 9am and 5pm, expectation that the hours and shift pattern will extend in the future as the service develops. Part time hours may be considered. Flexible working patterns will be considered.
