Type: 9-month fixed term/secondment, part-time options considered
Shift Pattern: Initially Monday-Friday, 9am-5pm, with potential extension to cover 8am-8pm, 7 days/week
The Home First Response Service focuses on working collaboratively across acute and community teams to support frail patients in returning home or to a homely setting, including hospital-at-home services. The physiotherapist will develop rehabilitation care plans, support early discharge, and prevent hospital admissions, working autonomously within the Renfrewshire HSCP structure. The role involves assessment, care planning, and working proactively with multidisciplinary teams to ensure seamless patient care.
Flexible working hours, with initial hours 9am-5pm, Monday to Friday, with future extension expected. Support for professional development and supervision is provided. The service aims to develop community skills and innovative care delivery.
