The Home First Response Service is a new initiative aimed at working collaboratively with acute and community teams to identify frailty and support patients to return home or stay in a homely setting. The physiotherapist will work within the RAH, focusing on community inreach, with supervision supported by the Renfrewshire HSCP. The role involves working at the hospital's front door, collaborating with ED, assessment units, and frailty services to improve patient outcomes through community interventions that facilitate quick discharge home. The position is managed under the Renfrewshire HSCP management structure. Candidates with an interest in Quality Improvement and Service Development are encouraged to apply.
