Job description

Hospital at Home service is looking for either an Occupational Therapist or Physiotherapist to become a member of the multidisciplinary team as part of an improvement project developing the Frailty services in Dumfries and Galloway.
The Hospital at Home (H@H) service provides specialist comprehensive assessment and care to older adults with frailty in the acute hospital and their own homes during an acute phase of illness where it is safe and appropriate to do so. This is a developing service in Dumfries and Galloway and supports the Health and Social Care Partnership’s aim of helping people stay at home, prevent unnecessary admission to hospital and improve patient outcomes.
You must be a good team worker but also be able to work independently without direct supervision. The role requires an individual who is forward thinking and motivated to actively participate in this improvement project to develop the role of AHP in the service.

You will work collaboratively with, and be supported by, the Frailty team leads, Consultant Geriatricians, GPs and a team of nurses with professional support provided by the AHP Manager. The team are looking to expand and support MDT working to enhance the holistic approach for the service user. You will work within the acute hospital setting and service users homes, assessing older people and supporting their carers, family and support network with. This post would suit an Occupational Therapist or Physiotherapist with experience of elderly care, emergency medicine and/or a rehabilitation background, but we would welcome applications from a variety of clinical backgrounds.

CPD is encouraged, allowing you time and resources, as appropriate, to support your learning growth development. The service offers a friendly and supportive working environment which actively supports Continuing Professional Development, through an established supervision, appraisal system (TURAS) and in-service training.

Training will be provided to ensure that you are able to complete all aspects of the role under the guidance of a senior clinician. The Hospital at home team operates 5 days a week, currently with the potential to expand to 7 day working model. The post holder will be required to work within these operating hours, including sharing weekend and bank holiday cover with the other members of the team where appropriate. Depending on the profession they will be expected to support the
Occupational Therapy weekend working or Physiotherapy weekend and on call service as skill level allows.
