Job summary

Epsom General Hospital in Epsom is recruiting a Home First Senior Clinician (Band 8a) on a 12-month fixed-term contract. The post is full-time (37.5 hours per week) and operates 8am–8pm, 7 days a week, under Surrey Downs Health & Care and Epsom & St Helier University Hospitals NHS Trust.

You will provide autonomous clinical assessment and management, work with a multidisciplinary team, and contribute to developing the Home First/UCR virtual ward model in patients' homes.

Qualifications

• Registered health care professional with relevant professional qualification.
• Completed Master’s Degree in Advanced Practice or working towards.
• Non-medical independent prescribing for healthcare professionals or.
• Management of patients with acute medical illness.
• Understanding of local and national policy as well as wider health economy.
• Demonstration of leadership or management experience.

Responsibilities

• Provide healthcare independently to patients, accountable for the total episode of care.
• Deliver advanced practice, work autonomously in assessment, diagnosis, planning and discharge.
• Collaborate with the multidisciplinary team and contribute to service development.
• Build relationships with Care Homes and support the Home First model.
