Job summary

Surrey Downs Health and Care in Surrey is seeking a practitioner to assess and treat older adults with complex health and social needs, referred from the community or identified in the Emergency Department and Acute Medical Unit. The role focuses on preventing admission and supporting safe discharge with holistic care planning and leadership.

You will use advanced physical assessment, work autonomously within professional boundaries, and lead service development within the Home First

Qualifications

• Advanced physical assessment is required.
• Current registration with HCPC or NMC is required.
• Experience assessing and managing the needs of complex patients in community or unscheduled care.
• Ability to work under pressure, balance priorities and meet deadlines.

Responsibilities

• Assess, diagnose, treat and review adults referred from the community, from initial history to evaluation of care
• Find and assess suitable patients in the Emergency Department and Acute Medical Unit as required
• Deliver specialist care for older people with complex health and social needs, both new referrals and existing caseload
• Carry out proactive holistic assessments and care plans to reduce the risk of future hospital admission
• Use critical thinking and safe clinical decision-making, working autonomously within professional boundaries
• Support supported discharge and admission prevention within the Home First multidisciplinary team
• Coordinate a seamless service through improved interdisciplinary processes and communication across Home First and the wider Surrey Downs Health and Care system
• Lead and develop services in line with patient need
• Act as a professional role model for internal and external colleagues
• Support the delivery of policy and procedures, providing leadership
• Undertake the clinical, research and development and leadership responsibilities set out in the attached job description
