Job summary

Surrey Downs Health and Care is seeking a Home First Senior Clinician (NHS AfC Band 8a) for a 12-month fixed term. The role is full-time at Epsom General Hospital, working 37.5 hours weekly within a 8am–8pm, 7 days a week service.

You will lead a multidisciplinary team to deliver rapid assessment and wrap-around home care for acutely unwell patients, aiming for early discharge when appropriate. The post requires registered professional status, an advanced practice degree (or ongoing study), and

Qualifications

Registered health care professional with relevant professional qualification.
Master’s Degree in Advanced Practice or working towards.
Non-medical independent prescribing for healthcare professionals.
Experience in management of patients with acute medical illness.
Understanding of local and national policy and wider health economy.
Demonstration of leadership or management experience.

Responsibilities

Provide healthcare independently to patients using specialist knowledge; accountable for the total episode of care.
Deliver advanced practice, act as role model, and plan care, treatment and safe discharge.
Work closely with multidisciplinary team and contribute to service development.
Build relationships with Care Homes and collaborate with the Quality & Care Home Liaison team.
