Surrey Downs Health and Care is seeking an experienced Clinician for the Home First Service in Epsom. You will lead clinical assessment, diagnosis, treatment, and evaluation of care for adults, working within a multidisciplinary team to provide safe, effective, person-centered care.

Qualifications

• Current registration with HCPC or NMC.
• Advanced physical assessment skills.
• Independent prescriber or working towards.
• Extended clinical skills or working towards.

Responsabilités

• Clinical responsibility for assessment and treatment planning for adults referred from community as well as active case finding within the ED and AMU.
• Coordinate a seamless service through enhanced interdisciplinary team processes and communication.

Current Registration with HCPC/ NMC
