Job summary

Gloucestershire Health and Care NHS Foundation Trust in Gloucester is recruiting a highly organised Home First Planner and Administrator to support the new countywide Home First service launched on 1 September 2026.

You will coordinate patient visits, manage capacity and demand, and provide comprehensive admin support to the multidisciplinary team, acting as a point of contact for staff, patients, carers and partner organisations.

Qualifications

• GCSEs in Maths and English at grade C or above.
• NVQ Level 3 in administration or equivalent.
• RSA Typing Level 2 or equivalent.

Responsibilities

• Coordinate and schedule patient visits to deliver care plans.
• Maintain schedules and monitor service capacity and workforce demand.
• Record information in clinical systems and manage diaries and meetings.
• Provide comprehensive administrative support to multidisciplinary teams.
