Job summary

The role involves flexible shifts across multiple sites, with emphasis on independence and safe, patient-centered support. Driving licence and access to a vehicle are required.

Qualifications

• GCSE level Pass or equivalent in Maths and English.
• Care Certificate.
• NVQ in Health & Social Care or related training.

Responsibilities

• Work alongside experienced nursing and multidisciplinary teams.
• Provide personalised care and reablement to service users.
• Undertake assessments, review progress and support independence.
• Collaborate with therapists, nurses, social care colleagues, families and carers.
