Job summary

Gloucestershire Health and Care NHS Foundation Trust has an exciting permanent opportunity for a Band 3 Home First Front Door role based in Gloucester. The post covers full-time or part-time hours (37.5 per week) with rota patterns across 7 days and shifts between 7am and 10pm.

You will be the first point of contact for referrals and enquiries, coordinating with Occupational Therapists, Physiotherapists and other professionals to support people to stay at home where possible.

Qualifications

• Experience working as part of a team.
• Experience communicating over the telephone.
• Ability to work unsupervised and manage time effectively.
• Good interpersonal skills and professional communication.

Responsibilities

• Act as the first point of contact for Home First referrals and enquiries.
• Manage incoming calls and referrals with accurate data recording.
• Support urgent referrals in collaboration with clinical leads.
• Direct referrals using agreed pathways and tools.
• Maintain patient records on clinical systems, including SystmOne.
• Coordinate with multidisciplinary teams to ensure timely care.
