The role focuses on operational leadership, workload planning, staff recruitment and development, service monitoring, safeguarding, and collaboration with partners across KEaH, social care, GPs, and discharge teams.

Qualifications

• Experience leading a multidisciplinary team of assessors, community support workers and admin staff.
• Ability to manage defined caseloads with timely assessments and discharge coordination.
• Commitment to safeguarding, patient safety and quality of care.

Responsibilities

• Operational Leadership: Oversee staff rotas and deployment across locality to meet demands.
• Case and Workforce Management: Manage a defined caseload with timely assessments and discharge coordination.
• Recruitment and Development: Lead on recruitment, induction and training; mentor and supervise staff.
• Service Monitoring and Reporting: Monitor activity levels, outcomes and referral trends.
• Quality, Safety, and Safeguarding: Maintain a safe environment and address safeguarding/incidents and concerns.
• Stakeholder Engagement: Collaborate with KEaH, social care, GPs and discharge teams to ensure seamless care.
• Innovation and Improvement: Identify service development opportunities and implement new models of care.
